Part time Case Manager

The Mission of Yahweh is a Non-Denominational Christian shelter for homeless women and children. We have served this population since 1961 and our goal is to help our residents achieve self-sufficiency through our emergency shelter and transitional housing programs. Our case manager is the person responsible for championing and counseling each resident during her quest to achieve self-sufficiency. The Case Manager interviews each client upon entry into our program, meets with the client to compile an action plan with achievable, measurable weekly goals, and monitors progress towards goals on a regular basis. Utilizing both in-house and outside service provider partnerships, the Case Manager leads each client through a succession of steps involving documentation, registration for publicly funded programs, personal counseling, and a host of on-site classes. Minimum requirements LSW, LBSW LMSW or Bachelors in a social service field with at least 1year case management experience preferably with the homeless population. Spanish speaking is preferred but not required
