The Alzheimer's Association Houston and Southeast Texas Chapter is searching for an individual with a high level of personal initiative and organizational skills to be responsible for Care Consultation and Support Group Coordination within the Chapter’s 29 county service area.
Responsibilities include assisting persons with dementia and/or their families in planning for and dealing with all aspects of the illness experience by assessing needs and developing an individualized action plan. Consultations provide families with information and appropriate referrals, supportive listening, and follow-up if necessary.
This position is also responsible for coordinating all affiliated support groups within our service area. Duties include, but are not limited to: providing initial and ongoing training for all support group facilitators, gathering data on support group members, and supplying facilitators with updates, information, and referrals.
Masters degree in Social Work or related field with at least two years experience required.
