Gateway to Care is a 501(c)(3) collaborative comprised of 170 public and private safety net health systems, coalitions, advocacy groups and social service providers working together to assist the uninsured and underinsured residents in the Greater Houston Area in receiving medical care at the most appropriate setting. Currently, we are searching for a Diabetes Focus Navigator to join our team! Responsibilities: - Provide assistance, training, guidance, direction and support to uninsured and underinsured persons with an emphasis on persons diagnosed with Diabetes. - Provide assistance to eligible persons in need of access to health care and social services for which they are eligible. - Document client information in Navigation Services database in a timely manner. - Follow-up with client and/or provider to ensure linkages to health care services are successful, especially to determine that all education is given and understood in regard to lifestyle and other changes required by the disease condition. - Network with appropriate community agencies and providers that have health care access programs. - Provide training to community organizations that partner with GTC and enrollment effort to provide identified staff the tools and ability to provide support to persons with diabetes. - Function independently and as a productive member of the team.
